Participant agreement
Assumption of risk and liability waiver
This agreement must be reviewed and signed electronically before any Living Pilates class, pass booking or waitlist request can be completed.
Voluntary participation
I voluntarily choose to participate in Pilates, barre, strength, mobility, stretching, foam rolling and related movement activities offered by Living Pilates LLC. I understand that these activities may be physically demanding and that participation is never required.
Health responsibility
I am responsible for deciding whether I am able to participate safely and for consulting a qualified health professional when appropriate. I will tell the instructor about relevant injuries, pregnancy, recent surgery, medical conditions or movement restrictions before class. Living Pilates does not diagnose, treat or replace medical care.
Assumption of inherent risk
I understand that movement activities and the use of Reformers, springs, platforms, weights, bands, balls and other equipment involve inherent risks. These can include muscle soreness, strains, sprains, falls, equipment-related injury, aggravation of a pre-existing condition, serious injury and, in rare cases, death. I knowingly accept the inherent risks of participation.
Release and responsibility
To the fullest extent permitted by California law, I release and hold harmless Living Pilates LLC and its owners, instructors, contractors and agents from claims arising from the ordinary inherent risks of my voluntary participation, except where a claim cannot legally be released. I remain responsible for loss or damage caused by my intentional misconduct or failure to follow reasonable safety instructions.
Emergency care
If I become ill or injured and cannot direct my own care, I authorize Living Pilates to contact emergency services and the emergency contact I provide. I understand that I am responsible for resulting medical or transportation costs.
Studio conduct and equipment
I will use equipment only as directed, wear required grip socks, keep walkways clear, and tell the instructor promptly about pain, dizziness, shortness of breath or equipment concerns. I understand that an instructor may modify or stop my participation when reasonably necessary for safety.
Electronic acknowledgment
Typing my name and selecting the required acknowledgment at checkout is my electronic signature. I confirm that I have read this agreement, understand it, have had the opportunity to ask questions and sign it voluntarily. Completing this agreement online means I do not need to sign the same version again at the studio. A parent or legal guardian must sign for a participant under 18.
Severability and California law
If a provision is found unenforceable, the remaining provisions continue to apply to the extent allowed by law. California law governs this agreement. This waiver does not waive any right that cannot legally be waived.
